- Reach out to our admin team at scheduling@flourishchristiancounseling.org with the following information:
- If patient is a minor: Parent’s first & last name, email address, and phone number
- Patient’s first & last name, date of birth, and email & phone if applicable
- Pictures of the front and back side of the member’s insurance ID
